This Child Health Pathways Project breaks down the barriers that both services and families are facing to enable direct family centred care and, where possible, remove barriers by providing practical support to families with children aged 2.5 – 5.5 years connected to the TOT’s Program in Logan with complex/multiple needs.

To achieve this, Gunya Meta’s Community Support Coordinator works alongside child health and specialist support case management services and clinical care coordination approaches.
